POLICY:Triangle Aftercare will designate a geographic service area within which the Company will accept client referrals.
PROCEDURES:
- Triangle Aftercare will accept client referrals within our geographic service area, which is generally defined as a 50 mile radius of each store. More specifically, the geographic service area is defined as all of
- Referrals outside the defined service areas will be declined. There may be very unusual, extenuating circumstances under which the General Manager would accept a referral outside of the Company’s service area. In such an instance, the General Manager would be responsible for documenting the reasons for such acceptance and a plan for the acceptable monitoring of that patient’s plan of care
